# Parental Consent to Survey Students Under 13
## Process for Gathering Guardian Consent for Student Pulse Checks
#### In compliance with [COPPA](https://www.ftc.gov/legal-library/browse/rules/childrens-online-privacy-protection-rule-coppa) (Children’s Online Privacy Protection Rule), Possip is committed to informing parents/guardians before surveying students under the age of 13. When working with customers who send out Student Pulse Check surveys, we are committed to protecting the privacy and safety of all personal information shared with us online by our survey participants. We always provide the option for families to Opt-Out of their children participating in surveys we administer for our partners.
*\*We also provide the option to Opt-Out of a Pulse Check at any point if a student no longer wishes to receive Pulse Checks. We offer anonymity with every Pulse Check to protect the privacy of our survey participants.*
Process for Gathering Guardian Consent for Student Pulse Checks

**Option A: Gather guardian consent through a survey**
If your organization does not yet have a process to ask for consent, here's an easy way to meet this requirement:
1. [Duplicate this form](https://docs.google.com/forms/d/14HGSpSKhxz3-NdDTJtFdY0wFEaAN6K60m-sMN-vp3CM/edit) to create your own version of a consent permission form.
2. Send this form to families and have those who do NOT consent to their children receiving the survey fill out the form.
3. Upload the finalized student contact list to your Documents tab on our platform.

**Option B: Gather guardian consent through a Family Pulse Check**
If you currently send Family Pulse Checks, you have the option to gather parent/guardian consent through a custom bonus question. You can discuss this option with your Customer Relationship Manager. We will generate our consent list based on how families respond to the Pulse Check.
Sample Bonus Question:
*Hi from ! We plan to survey scholars about their experience at school so that we can continue to improve. Surveys are always optional and can be done anonymously. We need parental permission for scholars under 13. If you have a scholar under 13, do you consent to them receiving Possip Pulse Checks? Please reply: YES, NO, or NA.*

**My organization already gathers guardian consent to survey:**
Your organization may already have a process that asks parents for permission to survey their children. If that's the case, please provide a student contact list with the following:
- First and Last Name of the Parent/Guardian
- Student ID ( Optional)
- Student First Name
- Student Last Name
- School Name
- Student Grade
- Student Email
- Student Language - ([Click here](https://hubs.ly/H0PZqYq0) for a list of languages Possip translates).
- Clear acknowledgement that YES, the guardian consents to their student receiving a survey
